My Last Wishes – Holy Rosary Lay Fraternity of St. Dominic

To Whom it may Concern: When my days are over and I am passing into Eternal Life, I would like these instructions carried out. I belong to the Dominican Laity Holy Rosary Chapter at Holy Rosary Church in Portland Oregon. Because of my membership in the Dominican Laity, I have the privilege to choose how I would like to be dressed on my final day. I have made provisions for this request and have selected below the option that I have chosen. *

Option 1 – A Burial Habit made just for this occasion. – A Belt that is to be worn with the Burial Habit and is with it. – A Burial 15-decade Rosary, that is also with my Burial Habit.
Option 2 – I prefer to be buried in clothing in the colors of black and white (or other, specify below) and will wear my large Dominican Cross on a wide black and white ribbon.
Option 3 – I prefer to have my family make these decisions….
